Levine-Fricke-Recon <br /> Section: 10 , <br /> Date Initiated: September 23, 1996 <br /> Revision No.: 1 <br /> Date Revised:August 21, 1998 <br /> Page 30 of 34 <br /> All employees who are injured due to overexposure involving hazardous <br /> substances. <br /> Each individual must have completed an annual surveillance examination and/or an <br /> initial baseline examination within the last 12 months. <br /> Personnel who wear respirators will be trained in their use and must have successfully <br /> passed a qualitative respiratory fit test in accordance with 29 CFR 1910.134 and <br /> 8 CCR 5144 within the last twelve months. <br /> A signed physician's statement qualifying the individual for hazardous waste operations <br /> will be required for each site worker as part of the medical monitoring program. <br /> Documentation of physician's approval, respirator fit-testing, and hazardous waste <br /> operations training will be maintained by the Project Manager for each worker at the Site. <br /> The medical requirements for selected subcontractors will be waived if they only ' <br /> participate in non-intrusive activities where exposure to affected media is not anticipated. <br /> Air monitoring data will be maintained by the SSO for review by site personnel.
