Levine-Fricke-Recon <br /> ' Section: 10 <br /> Date Initiated:September 23, 1996 <br /> Revision No.: 1 <br /> Date Revised:August 21, 1998 <br /> Page 29 of 34 <br /> Before the commencement of the project, the SSO will conduct a site safety briefing, <br /> which will include all personnel involved in site operations, including subcontractors. <br /> This meeting will include the following site-specific topics: <br /> 1. contents of this Health and Safety Plan <br /> 2. types of hazards at the Site and means for minimizing exposure to them <br /> 3. the type of monitoring that will be performed <br /> 4. action levels for upgrade and downgrade of personal protective equipment <br /> ' S. personal protective equipment that will be used <br /> 6. decontamination protocol <br /> 7. work zone, site control measures, including safe operating practices and <br /> communication and chain-of-command <br /> 8. emergency procedures <br /> 9. location and use of emergency equipment <br /> 10. evacuation signals and procedures <br /> In addition, the SSO will discuss the above topics with all personnel new to, or <br /> unfamiliar with, the Site before they will be permitted to perform any activity at the <br /> Site. <br /> 10.1.1 Daily Tailgate Safety Meetings <br /> A daily tailgate safety meeting will be conducted by the SSO before each day's <br /> activities to review pertinent safety issues, hazards anticipated during the day's <br /> ' activities, contingency procedures, and PPE to be worn. The meetings will be <br /> documented on the appropriate LFR Daily Tailgate Safety Meeting Forms. <br /> ' 10.2 Medical Monitoring Requirements <br /> ' All personnel, including subcontractors and site visitors, who will or may work in the <br /> Exclusion Zone of the Site must have fulfilled the appropriate medical monitoring <br /> requirements in accordance with 29 CFR 1910.120(f) and 8 CCR 5192(f). In general, <br /> ' these regulations require that medical surveillance be provided for employees who: <br /> • Are or may be exposed to hazardous substances at or above the permissible <br /> exposure limit, without regard to the use of respirators, for 30 or more days per <br /> year. <br /> • All employees who wear a respirator for 30 or more days a year. <br /> ' RIFSWP-AppF-Rev 1-0411I:SRL Page F-29 <br />
