PERMITTING AND PRE-FIELD WORK ACTIVITIES <br /> A site assessment boring permit will be obtained from the EHD for the boring location.As required <br /> by EHD permitting,a Site Health and Safety Plan will be prepared in accordance with Occupational <br /> Safety and Health Guidance Manual for Hazardous Waste Site Activities (National Institute for <br /> Occupational Safety and Health Administration)and with federal(29 CFR 1910.120)and state(title <br /> 8, Cal. Code Regs., section 5192) regulations. <br /> The SHSP will include: <br /> • Field activities, standard operating procedures (SOP); personnel requirements, potential <br /> hazardous substances, personnel w/ responsibilities, chemical, physical hazards, confined <br /> spaces and climatic conditions. The risks arising from the work being performed: impact to <br /> workers, community and the environment; Exposure monitoring plan; personal protective <br /> equipment and engineering controls; Site controls including work zones and security <br /> measures/ decontamination procedures. <br /> • Overall workpractices;with sanitation facilities/plans.Emergency response plan addressing <br /> hazardous material/waste releases; personnel training requirements; personnel medical <br /> surveillance program/record keeping. Requirements that all field personnel will be trained <br /> in hazardous materials communication and will have completed a 40-hour Health and Safety <br /> Course as specified in OSHA Hazardous Waste Operations and Emergency Response <br /> Standard [29 CFR 1910.120(e)], and 8-hour annual refresher courses as necessary. <br /> • Prior to digging activities, each proposed digging location will be clearly marked and an <br /> utility clearance obtained through Underground Service Alert. <br /> Advanced GeoEnvironmental,Inc. <br />
