After <br /> reviewing the documentation we find the following additional information is still needed to determine <br /> your eligibility for placement on the Priority List: <br /> • Submit a map drawn to scale that shows all underground storage tank locations, a north arrow and <br /> distances relative to the nearest public road. On page two(2) of your claim application you identify <br /> two USTs(one 5,000 gallon UST &one 7,000 gallon UST). However, the map shows three USTs (a <br /> 12,000 gallon UST&what appears to be a split UST, 5,000 gallon&7,000 gallon). Please revise <br /> page two (2)to include all USTs (previous/present). <br /> • Copy of the first letter from the local regulatory agency naming you (E.R. Vine and Sons) a <br /> responsible party and directing you to cleanup the contamination at the subject site.' Two letters <br /> were provided. 1) San Joaquin County's letter dated 9/29/03 to MRM Enterprises regarding the <br /> reminder of under-dispenser containment requirement for all new UST installations; and 2) San <br /> Joaquin County's letter dated 9/16/03 to Mr.Richard Erickson/Donlee Pump Company, confirming <br /> the completion of the dispenser island soil investigation and removal (no further action). You must <br /> provide written documentation from the regulatory agency naming claimant responsible party and <br /> directing.you to cleanup the contamination. <br /> • Submit a copy of your first permit to own or operate the UST from the local regulatory agency(Air <br /> Pollution permits are not acceptable). <br /> • Claimants who acquire sites after January 1, 1990,must complete the enclosed Claimant Certification <br /> of Compliance with Fund Regulations Section 2811(a)(I)-(2)and Health& Safety Code, Section <br /> 25299.54, subdivision (h). Please provide this document with claimant's original ink signature(what <br /> you provided appears to be a copy). <br /> Additionally, this form indicates you installed the UST and received your first permit on April 7, <br /> 1992.
