(EAI) has been retained by Montgomery WardCo., <br /> Incorporated (Montgomery Ward) to complete subsurface assessment activities to def' e the <br /> extent of contamination associated with the operation of underground storage tank (UST) <br /> systems formerly located at the above-referenced site. The work is being completed pursuant <br /> to and in accordance with directions from the San Joaquin County Public Health Services, <br /> Environmental Health Division (County). The County identifies the Montgomery Wad site <br /> by Site Code 2163. Mr. Michael Infurna (360/468-3454) is the County staff person <br /> responsible for the site. <br /> The next phase of assessment work includes the installation ofone ground water <br /> monitoring well in the City of Stockton right-of-way along Pacific Avenue (see Figure 1). <br /> EAI was informed that a Revocable Permit and an Encroachment Permit are required by the <br /> City for this well. The following are submitted to obtain these permits: <br /> 1. EAI hereby requests issuance of a Revocable Permit to install one ground water <br /> monitoring well on the City of Stockton right-of-way at the location indicated <br /> on Figure 1. EAI further acknowledges having read, understood, and ag ees to <br /> the City's Standard Conditions Applicable to Revocable Permits. Attachment A <br /> contains an executed Revocable Permit Application. <br /> 2. Attached is a check made payable to the City of Stockton in the amount c f S310 <br /> to cover costs associated with issuance of the Revocable Permit. <br /> 3.
