In lieu of the Faithful Performance Bond, enclosed herewith is a check in the <br /> amount of $500 made payable to the City of Stockton. EAI was informed that <br /> the $500 will be refunded when and if the ground water monitoring ell is <br /> removed from the City's right-of-way. <br /> 5. Attachment C contains a copy of the certificate of insurance issued by EAI <br /> naming the City of Stockton as an additional insured. The original certificate <br /> was forwarded directly to the City of Stockton, Risk Management Divisiori. <br /> 6. The property adjacent to the proposed ground water monitoring well location is <br /> owned by Stone Bros. Stone Bros. (R. Jay Allen - 209/478-1791) has been <br /> notified that Montgomery Ward is planning to install this well in the City of <br /> Stockton right-of-way. <br /> 7. Attached hereto is a check in the amount of $128 made payable to the City of <br /> Stockton for issuance of the Encroachment Permit. <br /> EAI is a state of California licensed Class A General Engineering contractor (license <br /> /1606348) with a hazardous substances certification (see Attachment D).
