a
HEALTH SE&CES
PUBLI
SAN JOAQUIN COUNTY r.
Q:
ENVIRONMENTAL HEALTH DIVISION
Ernest M. Fujimoto, M.D., M.P.H., Acting Health Officer
445 N. San Joaquin Street • P. O. Box 388 • Stockton, CA 95201-0388
209/468-3420 CHARLES WEST ; Py MONTGOMERY WARDC
39201 FREMONT
FREMONT CA 94538 DECc2, ; 199 ,
RE: Montgomery Ward Auto Center SITE CODE: 163
5400 Pacific Ave
Stockton, Ca 95207
Thank you for submitting the "Work Plan for Phase VI Subsurface
Investigation" in a timely manner as directed in correspondence
dated September 9, 1994 . A review of the plan has been completed
by staff and San Joaquin County Public Health Services,
Environmental Health Division (PHS-EHD) has the following
comments .
Historical background information detailed within the workplan
described soil sampling collection in May of 1988, by A. D.
Selditch & Associates, Inc . Of the original five borings labeled
"Borings 1 through 511 , only the locations of the first four
borings were shown on the included site map. I was unable to
find the location of Boring 5 described in paragraph 1, page 2 of
the workplan as, " . . . located in the west corner of the Montgomery
Ward site. . . " . Please submit to PHS-EHD an updated map and any
pertinent laboratory data that may supply the "background soi
conditions" this boring was designed to provide .
PHS-EHD requests that summary tables of groundwater analytical
data included within your consultant' s reports also include a
column that shows the depth to water (DTW) on the same date the
samples were collected. There is substantial fluctuation in the
contamination levels from one quarter to the next and the DTW
information may shorten the research time needed to correlate the
data.
Currently there is no Site Specific Safety Plan (SSSP) on file
for your consultant and one will be required prior to the start
of any field activities . Please have Environmental Audit, Inc.
include a list of 24-hour phone numbers of the persons
responsible for field activities during this phase of
investigation and a map to the nearest emergency medical facility
with the route highlighted.
A Division of San Joaquin County Health Care Services
