I am the one that will process and issue the <br /> well destruction permit (required)as well as inspect the field procedures. <br /> In order to avoid any delays that this process could cause if not processed <br /> in time, I've attached the permit application (two pages) . Open and print the <br /> document. This is the form you will need to get the permit to destroy the <br /> well. It should be submitted to the EHD with a $60 check as soon as you can. <br /> The driller (C-57) is the one to fill out page two, the "LCD" . He can <br /> fax/email me that form after it is filled out. <br /> In addition to the application, I will need a/the work plan that details the <br /> procedures and materials to be used in the destruction process, an access <br /> agreement from the Bank or Stone Bros that allows you to do the work if one <br /> is needed/issued, and the CVRWQCB work plan/well destruction approval letter <br /> that Mr. Barton will generate. <br /> As for the work plan, our County Well Standards allow for multiple ways to <br /> destroy a well. Two are based on whether the well is 'contaminated' . I am <br /> unaware if MW-15's ground water or soil contained/contains contamination or <br /> not. Please provide me with the latest ground water sample data and ANY soil <br /> data (installation?) that could support whether or not contamination is <br /> present or not. Wells with contamination present must be over bored or wire- <br /> line destroyed only. Non contaminated wells may be pressure grouted if <br /> surface sealing procedures are followed. Non contaminated wells may also be <br /> over bored if you wish. According to Mike McDowell at the <br /> City of Stockton Permit Center (Department of Public Works) , the permits <br /> are in review for the improvements to the former Union Bank property <br /> referenced below. The improvements include street widening of Pacific <br /> Avenue that will almost certainly encroach upon groundwater monitoring <br /> well MW-15 located just north of the northwest corner of Pacific Avenue <br /> and Robinhood Drive (see attached figure) . According to Mr. McDowell <br /> the project is on an "accelerated schedule" and the business is likely <br /> to open in August 2007.
