Health and Safety Plan <br /> Cleanup and Abatement Order No 5-00-707 EAI Protect No 1232 <br /> 2.0 AUTHORITY AND RESPONSIBILITY <br />' The EAI Site Safety Officer (SSO) is responsible for the daily implementation of the health and <br /> safety program The SSO will be on-site during Site activities to monitor safety and health and <br /> decontamination protocols Prior to initiation of Site activities, the SSO, EAI Project Manager <br />' (PM) or qualified designee will verify that EAI personnel and subcontractors who will be on <br /> the Site have read, understood and agreed to the health and safety procedures outlined herein <br /> The SSO and PM, individually, have the authority to take appropriate actions or stop work to <br /> ' achieve compliance with this HASP The EAI personnel assigned and their various roles are <br /> listed below <br /> ' Assignment Name Phone Number <br /> Project Director Steven Bright 714/632-8521, ext 224 <br /> ' Project Manager Boris Stolin 714/632-8521, ext 240 <br /> Site Safety Officer Brent Mecham 714/632-8521, ext 226 <br /> Field Team To be determined <br /> ' 2.1 Project Director <br /> The Project Director is responsible for the overall operation of the project The Project <br /> Director will also hold review and planning meetings as necessary with all technical staff, <br /> during which the current project progress, problems encountered, and future direction will be <br /> discussed <br /> 2.2 Project Manager <br /> ' The PM shall direct all on-site operations The PM may delegate all or part of these duties to a <br /> properly qualified designee At the Site, the PM assisted by the SSO, has responsibility for <br /> ' • Ensuring that appropriate personnel protective equipment and monitoring equipment is <br /> available, in good working condition, and properly utilized <br /> • Establishing that personnel are aware of the provisions of this HASP, are instructed in <br /> the work practices necessary to ensure safety, and are familiar with planned procedures <br /> ' for dealing with emergencies <br /> `Establishing that Site personnel have completed a minimum of 40 hours of health and <br /> safety training and have appropriate medical clearance as required by 29 CFR 1910 120, <br /> W and have been fit tested for the appropriate respirators, if respirator use is warranted <br /> ' e Ensuring that Site personnel are aware of the potential hazards associated with their job <br /> function <br /> o Monitoring the safety performance of Site personnel to see that required work practices <br /> are employed <br /> ' <br /> SAB 1232CAOIIASP -3- <br /> ENVIRONMENTAL AUDIT,INC., <br />
