T Stockton RWCF, Tertiary Plantes -3- *0/ 14 Febrdary 2001 <br /> Inspection Report <br /> CONCLUSION AND RECOMMENDATIONS: <br /> The inspection of the Tertiary Treatment Facility did not reveal any obvious violations of the NPDES <br /> permit at the time of the site visit. The leaking recirculation gate valve has been repaired and a <br /> procedure is in place to prevent the possibility of leakage in the event of another broken valve nut. <br /> The release was an operational upset caused by mechanical failure and resulted in an unintentional <br /> bypass non-disinfected secondary effluent. The leak resulted in an estimated release of between 4,920 <br /> gallons to 14,400 gallons of non-disinfected secondary effluent,which was heavily diluted by the <br /> disinfected effluent. <br /> I also recommend that RWCF staff review and be familiar with standard operating procedures dealing <br /> with incident response. Standard Provisions and Reporting Requirements A.17. states in part, <br /> "Reasonable steps shall include such accelerated or additional monitoring as necessary to determine <br /> the nature of the impact of the non-complying discharger or sludge use or disposal." <br /> I recommend that future site inspections of the RWCF Tertiary Treatment Facility include examination <br /> of the amount of water in the open observation box to determine if the standard operating procedure is <br /> being implemented. <br /> Figure 1, the OES Hazardous Materials Spill Report and Photo Log complete this report. <br /> Approved: ROBERT S.
