Mr. James L.L. Barton,
ARCADIS P.G.
May 29, 2009
conjunction with the USA, a private utility locating company will be utilized to further
evaluate boring locations relative to underground utilities.
Well Destruction Activities
Pressure Grout and Over Drill
Prior to drilling, a minimum of three points, arranged in a triangle, will be cleared
around each well using either a hand auger or air knife to.a maximum depth of 5 feet +l /
1 inch bgs to visually inspect for the presence of utilities. Monitoring wells MW-2, V
MW-3, MW-4, MW-5, MW-6, MW-7, MW-8, MW-9, MW-10, MW-11 D, MW-12, MW-
13, and MW-14 will then be destroyed by removing the well box and pressure
grouting each well in-place from the bottom of the borehole to surface grade using a
tremie pipe and cement-bentonite grout. Per SJCEHD guidelines each well will be
grouted to form a"mushroom cap"at the surface of the borehole. The top three feet
will then be over-drilled with 12 inch outer diameter(OD)hollow stem auger(HSA).
The upper 5 feet will be backfilled with granulated bentonite chips and completed at
grade with material matching the existing surface(e.g. asphalt, concrete, etc.).
Soil Cuttings and Rinsate Water
Soil cuttings generated during drilling operations will be containerized in 55 gallon
drums or stockpiled and temporarily stored on the subject property pending
characterization and disposal. Soil cuttings will be removed by Chevron's disposal
contractor to an appropriate disposal facility.Water generated during steam cleaning,
i
and sampling of the borings-will be temporarily_stored in DOT-approved 55 gallon
drums pending transport by Chevron's disposal contractor to an appropriate disposal
or treatment facility.
Reporting
A report will be prepared to document the results of the destruction of onsite.and
offsite monitoring and extraction.wells and the decommissioning of the remedial
system. The report will include ata minimum the following:
• a summary of site conditions and background information j
I
• a scaled site plan illustrating the destroyed well locations andotherrelevant- -
site features
Page:
llxcadis-uslofficedalalSacramento-CA1ENVICHEVRONt9617116 OeliverahlesMork Plant200919617.1 Well Destruction Work Plan 333910950.doc 4/5
