(HFA) has prepared this work plan addendum for well destruction for the <br /> above-referenced site (Site). Previously, a work plan for well destruction dated May 29, 2009, a <br /> revised work plan for well destruction dated June 18, 2009, and a work plan addendum for well <br /> destruction dated August 12, 2009, were submitted for the Site. <br /> Based on conditions observed in the field during the geophysical survey and hole clearance <br /> activities, the previously approved well destruction method of complete overdrill is not <br /> appropriate for all wells. HFA recommends abandoning the following wells by pressure grouting <br /> utilizing the procedure outlined in the work plan addendum dated August 12, 2009. <br /> • MW-4: two polyvinyl chloride (PVC) conduits were encountered within 6 inches of the well <br /> casing during hole clearance activities. These PVC conduits most likely supply electricity and <br /> water to the site's air and water station. Use of rotating equipment in close proximity to <br /> these facilities would present an unacceptable safety risk. As such, pressure grouting is <br /> recommended for abandonment of well MW-4. <br /> • MW-8: an unknown fiberglass line was encountered within 6 inches of the well casing during <br /> hole clearance activities. Use of rotating equipment in close proximity to this line would <br /> present an unacceptable safety and environmental risk. As such, pressure grouting is <br /> recommended for abandonment of well MW-8. <br /> • MW-10: during the geophysical investigation, an electrical line was identified within 1 foot of <br /> the wellbox. Additionally, the wellbox is within 7 feet of the surface expression of the Site's <br /> current-generation USTs.
