7. The following information should be available when placing the <br /> notification call : <br /> a. Name of the facility. <br /> b. Location ( including the latitude and longitude) . <br /> c. Name and telephone number of the person in charge of the facility. <br /> d. The population density within a one-mile radius of the facility <br /> (described in terms of 0-50 persons, 51-100 persons, 101-500 persons, <br /> 501-1 ,000 persons, more than 1,000 persons) . <br /> e. The identity and location of sensitive populations and ecosystems <br /> within a one-mile radius of the facility. <br /> f. The name/identity of the hazardous substance, the chemical abstracts <br /> service registry number for the substance, if a mixture, the components of <br /> the mixture, their approximate concentrations and quantities, Chris Code, <br /> Un/DOT Number. <br /> g. The source(s) of the release. <br /> h. The environmental medium(s) affected by the release of surface water, <br /> the name of the surface water body, if a stream, its order or average flow <br /> rate ( in cubic feet/second and designated use, if a lake, the surface area <br /> ( in areas) and average depth, if on or underground, the location of any <br /> public water supply wells within two miles. <br /> i . The date and time of the spill/release. <br /> j . The quantity (lb, bbl , gal) . <br /> k. Damages, number of injuries, deaths, property damage >$50,000.00. <br /> 8.
