SPILL REPORTING GUIDELINES (40CFR 112.4) <br /> A written report must be submitted whenever the facility has: <br /> 1. Discharged more than 1,000 gallons of oil into or upon the navigable <br /> waters of the United States or adjoining shorelines in a single spill event, <br /> or <br /> 2. Discharged more than 42 gallons of oil in two spill events, occurring <br /> within any 12 month period, or <br /> 3. Discharged into navigable waters of the United States causing film, sheen, <br /> discoloration, or violates any applicable water quality standards (to be <br /> reported to National Response Center within 15 minutes of the person in <br /> charge of the facility has become knowledgeable of discovery). <br /> Given such spills, the responsible management will, within 60 days, submit to the United <br /> States Environmental Protection Agency Regional Administrator (US, EPA, RA) and the <br /> State Department of Fish and Game, the following: <br /> 1. Name of the facility <br /> 2. Name(s) of the owner or operator of the facility <br /> 3. Location of the facility <br /> 4. Maximum storage or handling capacity of the facility and normal daily <br /> throughput <br /> 5. The corrective actions and/or countermeasures taken, including an <br /> adequate description of equipment repairs and/or replacements <br /> 6. Adequate description of the facility, including maps, flow diagrams, and <br /> topographical maps <br /> 7. The cause(s) of the discharge, including a failure analysis of the system or <br /> subsystems in which the failure occurred <br /> 8. Additional preventive measures taken or contemplated to minimize the <br /> possibility of recurrence <br /> 9. Such other information as the Regional Administrator may reasonably <br /> require pertinent to the Plan or discharge <br /> 5411599 <br />
