Submit a work plan addendum for a properly designed monitoring well that meets <br /> San Joaquin County Well Standards to this office no later November 17,2000. <br /> • The current well permit application for the Geoprobe portion of the work plan submitted by Barber was <br /> not signed by the G57 licensed contractor,Fisch Environmental(Fisch). PHS/EHD also reminded <br /> Barber that a separate well permit application and additional fee of$89.00 must be submitted to this office <br /> if the monitoring well is to be installed by a different G57 contractor than Fisch. Submit an additional <br /> well permit application and appropriate fee(if required)to this office by November 24,2000, <br /> • Barber agreed to analyze both soil and groundwater samples for Total Petroleum Hydrocarbon(IPH)- <br /> Extractable as Diesel by EPA Method 8015K TPH as Kerosene by EPA Method 8015K TPI-Pursuable <br /> as Gasoline by EPA Method 8015K and Benzene,Toluene,Ethylbenzene,and Total Xylenes (BTEX <br /> collectively)by EPA Method 8020,as recommended by PHS/EHD in a letter dated November 9,2000. <br /> • PHS/EHD agreed to extend the schedule to perform the Geoprobe portion of the work plan from the <br /> week of November 20thto the week of November 27th. <br /> • Barber stated that he might have difficulties in obtaining a G57 licensed contractor to install the <br /> monitoring well on a short notice.PHS/EHD highly suggested Barber to start scheduling a G57 licensed <br /> contractor to do the work now. PHS/END requires the monitoring well be installed by December 15, <br /> 2000.
