Training was last conducted on 1/3/17 and has expired. Personnel who handle <br /> hazardous waste shall be trai ed within six months of assignment and take part in an annual review of the initial <br /> training received.This progra shall be directed by a person trained in hazardous waste management procedures. <br /> At a minimum,the training pr gram shall be designed to ensure that facility personnel are able to respond <br /> effectively to emergencies by amiliarizing them with emergency procedures,emergency equipment, and <br /> emergency systems, including where applicable: <br /> 1. procedures for using, inspecting, repairing,and replacing facility emergency and monitoring equipment; <br /> 2. key parameters for automatic waste feed cut-off systems; <br /> 3. communications or alarm systems; <br /> 4. response to fires or explosions; <br /> 5. response to ground-wa er contamination incidents; and <br /> 6. shutdown of operations <br /> The owner or operator shall aintain the following documents and records at the facility: <br /> 1. the job title for each po ition at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2, written job description f r each position listed above; <br /> 3. written description of th a type and amount of both introductory and continuing training that will be given to <br /> each person filing positions IiE led above; <br /> 4. records that document at the training has been given to and completed by facility personnel who is trained <br /> in hazardous waste managerT ent procedures.. <br /> Training records on current er i ployees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for a least three years from the date the employee last worked at the facility. Immediately <br /> provide employees with initial r refresher hazardous waste training. Submit a copy of the training records with the <br /> requirements listed above to t e EHD. <br /> This is a Class II violation. <br /> 119 HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of hazardous waste disposal records for the last three years were not found on site for the parts washer <br /> waste.According to the manager,Xavier Hernandez,the parts washer is routinely serviced when it gets full. <br /> Hazardous waste generators hall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifel e ting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of II manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> This is a minor violation. <br /> FA0009997 PRO514125 SCO01 02/27/2018 <br /> EHD 22-02 Rev.09/22/16 Page 6 of 10 Large Quantity Hazardous Waste Generator OIR <br />
