All -opies of each receipt shall contain all of the following information: <br /> -The name, address, idem ification number, contact person, and telephone number of the generator, and the <br /> signature of the generator ort a generator's representative; <br /> -The date of the shipment <br /> -The manifest number; <br /> -The volume or quantity oi each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/N )identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, a ddress, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by th B generator,certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicit of the hazardous waste to the degree,as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a minor violation. <br /> 301 CCR 66265.31 Facility not ma ntained to minimize the release of a hazardous waste. <br /> In the lube oil area,a white po y 55 gallon container of antifreeze was observed with a hole in the lip. Antifreeze was <br /> observed going through the hc le,going down the container, and pooling at the bottom. Staining was also observed <br /> on the floor of the shop aroun the bottoms of the drums and to the edge of the shop door. <br /> In the tune-up area, metal grin is were observed on the floor spread around the brake grinder. <br /> Facilities shall be maintained a nd operated to minimize the possibility of a fire,explosion, or release of hazardous <br /> waste to air,soil, or surface water which could threaten human health or the environment. Immediately clean the <br /> areas and manage according to Title 22 hazardous waste regulations.
