Absorbent used on spills were left on the floor of the work shop and in the garage. Facilities shall <br /> be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br /> waste to air, soil, or surface water which could threaten human health or the environment. Immediately <br /> clean up and manage according to Title 22 hazardous waste regulations. Submit a statement and <br /> supporting documentation explaining how this waste was managed by 11/10/2012. <br /> 404. Three 55-gallon drums of anti-freeze waste were not closed. All hazardous waste containers shall <br /> be closed at all times except when adding or removing waste. Immediately close these containers and <br /> ensure all hazardous waste containers are closed when not adding or removing waste. <br /> 605 & 701. Three 55-gallon drums of anti-freeze waste by the waste oil tank had no hazardous waste <br /> label, and the hazardous waste label for the waste oil tank was attached to the wall instead of the tank. <br /> All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> And All hazarous waste tanks shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. accumulation start date <br /> Immediately label these containers and the tanks ensure that they marked with all the required <br /> information. Since the three containers had no hazardous label, it is therefore considered to have been <br /> on site longer than 180 days. Immediately contact a licensed hazardous waste hauler to dispose of the <br /> waste in the three containers under manifest and submit a copy of the manifest to the EHD by <br /> 11/10/2012. <br /> 703. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose.
