SUMARY OF V LATIONS CLASS I, CLA , o INOR-Notice to Com This is a report of the Routine hazardous waste inspection conducted at the referenced facility by San Joaquin County Environmental Health Department (EHD). The following are Class II (Repeat) Violations: 407, and the following are Minor Violations - Notice to Comply- 101, 126, 101. This facility's EPA ID number CAC000282809 is inactive and registered under a different name: Gotelli Trucking. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately obtain a valid EPA ID number to manage hazardous waste and submit evidence to the EHD by 05/08/2012. A copy of the EPA ID number application form has been provided. 126. Consolidated manifests of used oil and anti-freeze generated at this facility used erroneous address of 1627 Channel. Consolidated manifests shall contain information in accordance to Health and Safety Code (HSC) 25160.2(b)(4), including name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator's representative. Immediately begin managing consolidated manifests per this section. 407. (Repeat) Observed 4 empty 55-gallon hazardous waste or material drums south of 1634 Channel building with no date of when it first became empty. Containers larger than five gallons that previously held a hazardous material shall be marked with the date they were emptied and managed within one year of being emptied. Immediately mark and manage all empty containers per this section. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
