If"No", identify the tank, describe the issue and actions taken in the Note Section below.
29. Are all valves free of leaks, corrosion and other damage?
29.1 Anti-siphon valve
29.2 Check valve
29.3 Gate valve
29.4 Pressure regulator valve
29.5 Expansion relief valve
29.6 Solenoid valve
29.7 Fire valve
29.8 Shear valve
30. Have the manufacturers' instructions for regular maintenance of these items been performed?
30.1 Anti-siphon valve
30.2 Check valve
30.3 Gate valve
30.4 Pressure regulator valve
30.5 Expansion relief valve
30.6 Solenoid valve
30.7 Fire valve
30.8 Shear valve
31. Are any strainers or filters that are present clean and in good condition?
Tank Manways, Piping,and Equipment
❑ ❑ ❑ 32. Visually inspect all tank top openings including nipples, manways, tank top overfill containers,
and leak detection tubes. Is the sealant between all tank top openings and concrete intact and
in good condition?
❑ ❑ ❑ 33. Are flanged connection bolts tight and fully engaged with no sign of wear or corrosion?
Piping, Hoses, Ladders, Platforms
34. Are piping connections to the tank(valves, fittings, pumps, etc.) free of visible leaks?
35. Do any rubber hoses look as if they will function problem free for a full 12 months from now?
❑ ❑ ❑ 36. Do the ladders/platforms/walkways appear to be secure, with no cracks, or sign of severe
corrosion, or other damage?
Spill/Fill Buckets/Strainers/Filters
37. Is this device in working order?
38. Is this device lockable?
39. Are strainers and filters clean and in good condition?
Tank Monitoring and Leak Detection Equipment
40. Is interstitial leak detection equipment in good condition?
41. Are all the wire connections intact?
42. If equipment has a test function, does it activate to confirm operation?
❑ ❑ ❑ 43. Manually gauge tank if necessary. Is liquid level shown by Tank Monitoring display consistent
with the actual amount in the tank?
❑ ❑ ❑ 44. If liquid level monitoring equipment is dependent on batteries, does the battery indicate ade-
quate charge to last 12 more months? If no or unknown, change batteries.
Overfill Protection
45. Are windows on sight gauges clear?
46. If overfill protection is dependent on batteries, does the battery indicate adequate charge?
47. Is liquid level, overfill equipment, and leak detectors in good condition?
48. Is the electrical wiring and boxes in good condition?
