Estimates of the total quantity discharged/spilled <br /> 8. The source of the discharge/spill (pipe, tank, truck) <br /> 9. The cause of the discharge/spill <br /> 10. A description of all the affected contaminated areas <br /> 11. Any damages or injuries caused by the discharge/spill <br /> 12. Actions being taken to stop, remove, contain and mitigate the effects of <br /> the discharge/spill <br /> 13. Whether or not an evacuation is warranted or needed <br /> 14.
