c. The EPA identification number, name, and address of <br /> the generator and the transporter who transported the <br /> waste; <br /> d. The license number of the vehicles used to transport <br /> the waste. This shall include the license number of <br /> the tractor as well as the trailers if appropriate; <br /> e. A description and quantity of the received or rejected <br /> load of hazardous waste; <br /> f. For waste received, the method of treatment, storage, <br /> or disposal for each hazardous waste; <br /> g. If rejected, a brief explanation of why the waste was <br /> rejected; <br /> h. A brief explanation of why the waste was unmanifested, <br /> if known; and <br /> i. A certification as required by item II.H of this <br /> permit. <br /> 4. Used oil shall be manifested under either one of the <br /> following procedures: <br /> a. The procedures prescribed in conditions III.G.1, <br /> III.G.2, and III.G.3. <br /> b. The following modified manifesting procedures, which <br /> may be used only- by a registered hazardous waste ' <br /> hauler and shall be used only with the consent of the <br /> generator: <br /> (1) A separate manifest shall be completed by each <br /> vehicle driver, with respect to each transport <br /> vehicle operated by that driver for each date. <br /> (2) An EPA identification •number_rss not required for <br /> an individual generator. rx <br /> (3) The hauler shall complete both, tHier_ggnerator;-s <br /> and the hauler's sections ,of the'; mah;i-f-est using <br /> the hauler's name. The hauler's section shall be <br /> completed prior to commencing each day's u 'ed oil <br /> collections. The driver shall sign and date the; <br /> generator's and the hauler's sections cf the' <br /> manifest. <br /> (4) The hauler's name, EPA identification number, an , <br /> terminal address shall be entered in the <br /> generator's name, address, and EPA identification <br /> number spaces of the manifest. <br /> —14— <br />
