Manifest num ers 007884265JJK, 006998400JJK, 006069145JJK, and 006068324JJK <br /> were not found on site. Ha ardous waste generators shall retain copies of all manifests for three years <br /> or until a copy signed off by the destination facility is received. The signed off copy shall be retained and <br /> made readily available for r view for three years. If the generator did not receive a copy of the manifest <br /> with the handwritten signatu a of the owner or operator of the facility to which the generator's waste was <br /> submitted within 35 days of he date the waste was accepted by the initial transporter, the generator <br /> shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste. If the g nerator did not receive a copy of the signed off copy of the manifest within <br /> 45 days of the date that the aste was accepted by the initial transporter, the generator shall submit an <br /> Exception Report that includ s a legible copy of the missing manifest, and a cover letter signed by the <br /> generator explaining the eff rts to locate the hazardous waste and the results of those efforts. <br /> Exception reports shall be s bmitted to: <br /> DTSC Report Reposi ory <br /> Generator Informatio Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy o the missing manifests or prepare and submit an exception report to DTSC. <br /> By August 26, 2011, submit o the EHD a copy of the signed off copy of the manifests or a copy of the <br /> exception report submitted t DTSC. <br /> 60 & 63. Copies of hazardous waste disposal records for July 26, 2008 to the present were not found on <br /> site. Hazardous waste gene :tors shall retain copies of all manifests signed off by the disposal facility <br /> and all receipts used in a co solidated manifesting procedure on site for three years and have them <br /> readily available for review. mmediately locate a copy of all manifests and receipts for the last three <br /> years, maintain them on site, and submit copies to the EHD by August 26, 2011. Include all disposal <br /> records for waste oil, waste oolant, waste absorbent, oil filters, parts washer waste, waste aerosol <br /> cans, brake rotor shaving w ste, and oil-water separator waste. <br /> Notes <br /> 1. Complete and submit a c py of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected including supporting documentation, by August 26, 2011. <br /> 2.
