CONTINUATION FORM <br /> OFFICIAL INSPECTION REPORT <br /> Facility Address: 1645 Cherokee, Stockton (CCT) Page 3 of 5 <br /> Date of Inspection: 12/07/2009 <br /> Date of Final Inspection Report: 12/08/2009 <br /> SUMMARY OF VIOLATIONS <br /> Class II (Repeat) Violations: # 20, 21 , 27, 33 <br /> #20 No Hazardous Waste (HW) label on one 5-gallon bucket and the collection <br /> bin used to contain drained used oil and grease. All HW containers must <br /> be labeled at all times at minimum with the following information: <br /> 1) Words "Hazardous Waste" 2) Generator name & address <br /> 3) Composition (contents) 4) Physical state <br /> 5) Hazardous properties 6) Accumulation start date <br /> Immediately label all HW containers as above, and continue this practice <br /> for all HW containers in the future. Submit evidence of correction to this <br /> office by January 8, 2010. <br /> #21 The above-mentioned bin was not close. HW containers must be closed <br /> at all times except when adding or removing HW. Immediately close all <br /> HW containers except when adding or removing, and continue this <br /> practice in the future. Submit evidence of correction to this office by <br /> January 8, 2010. <br /> #27 Disposal receipts of HW were not available for inspection. A generator is <br /> required to retain at least 3 years of disposal receipts of HW available for <br /> inspection on site. Submit a copy of all the disposal receipts of HW for the <br /> last 3 years to this office by January 8, 2010. <br /> #33 (3) A black metal 55-gallon drum of used oil filters was not close and <br /> labeled with an initial date of accumulation. Used oil filters shall be stored <br /> in closed, rainproof containers capable of preventing spillage of the <br /> drained used oil when laid upon their sides. The container must be <br /> labeled "Drained Used oil Filters" with an initial date of accumulation. <br /> Immediately close the container to prevent any spillage of drained oil, and <br /> label the container "Drained Used Oil Filters" with an initial date of <br /> accumulation. Continue this practice in the future. Submit evidence of <br /> correction to this office by January 8, 2010. <br /> (5) No bills of lading for filters for 3 years were available for inspection. <br /> Used filters may be stored on site no longer than one year from date of <br /> accumulation, and a record of transfer to a receiving facility must be <br /> retained on site for a period of three years. Immediately manage the <br /> used filters if they had been stored on site for more than one year. Submit <br /> a copy of all disposal receipts for the last 3 years to this office by January <br /> 8, 2010. <br />
