Mr. <br /> Lush had questions regarding the State Water Resources Control Board's <br /> Cleanup Fund (SWRCB) reimbursement process and he was referred to George <br /> Lockwood at SWRCB. <br /> PHS/EHD received a remediation proposal/work plan prepared by LUSH dated <br /> November 15, 1995. The work plan was approved by this agency with some <br /> changes on December 5, 1995. Lush was notified of the approval and PHS- <br /> EHD's permit requirements. The proposed scope of work was prepared as <br /> delineated in the Final Remedial Plan prepared by Geological Audit Services <br /> with some modifications. The corrective action plan to utilize vapor extraction to <br /> remediate the gasoline contamination at the site remained the same as <br /> proposed in the Final Remedial Plan approved by PHS-EHD in August of 1995. <br /> On February 21, 1996 PHS-EHD received two (2) permit applications submitted <br /> by LUSH and permits were issued on March 6 and 7, 1996. On site and off site <br /> field work was then performed. <br /> A report prepared and submitted by LUSH dated May 30, 1996 was received <br /> and reviewed by this office. In the report it stated: "Permitting, design, and <br /> construction of a remediation system was in progress". <br /> PHS-EHD received an additional report prepared by LUSH dated June 21, 1996 <br /> which stated: "Permitting and installation of vapor extraction system is ongoing. <br /> A Division of San Joaquin Countv Health Care Services <br />
