A vapor extraction unit was on site but plumbing to it <br /> had not been completed and the vapor extraction system was inoperable. <br /> On October 18, 1996 we met on site to review the problems you were <br /> experiencing with lack of performance by LUSH. At that time, the remediation <br /> system was still lacking plumbing and not operating while gasoline <br /> concentrations continue to increase in groundwater from the on site release. <br /> Several status reports prepared by you documenting the problems with the <br /> project were received by this agency. <br /> On February 28, 1997 PHS-EHD was notified of your change of consultants. <br /> Advanced GeoEnvironmental Incorporated (AGE) was awarded the operation <br /> and maintenance contract for the on site remediation system. AGE brought to <br /> the attention of this office substandard materials used by LUSH for system <br /> installation.
