Page 2
Chapin Brothers Incorporated
We anticipate the system will be in and operating within 30 to 60 days,
depending on permit requirements".
The last report submitted to this agency by LUSH was dated July 12, 1996.
Again, the report stated: "Currently, permitting, design, and construction of a
remediation system utilizing vapor extraction/air sparing is in progress".
On July 18, 1996 PHS-EHD performed a site visit to determine remediation
system equipment status. A vapor extraction unit was on site but plumbing to it
had not been completed and the vapor extraction system was inoperable.
On October 18, 1996 we met on site to review the problems you were
experiencing with lack of performance by LUSH. At that time, the remediation
system was still lacking plumbing and not operating while gasoline
concentrations continue to increase in groundwater from the on site release.
Several status reports prepared by you documenting the problems with the
project were received by this agency.
On February 28, 1997 PHS-EHD was notified of your change of consultants.
Advanced GeoEnvironmental Incorporated (AGE) was awarded the operation
and maintenance contract for the on site remediation system. AGE brought to
the attention of this office substandard materials used by LUSH for system
installation. With necessary changes, AGE was able to complete construction
and brought the remediation system on line which LUSH was unable to do.
If you have any further questions regarding this matter, contact Harlin Knoll at
(209) 468-3442.
Donna Heran, REHS, Director
Environmental Health Division
Harlin Knoll, Senior REHS Margarei Lagorio
Site Mitigation Unit Supervisor
HK.-1b
cc: SWRCB, Cleanup Fund - Dave Deaner
Advanced GeoEnvironmental
