Compliance Statement RECEIVED E C C I VE D <br /> 2014 ` 9� . <br /> JUN 6 2014 <br /> ENVIRONMENTAL HEALTH <br /> DEPARTMENT <br /> On May 6th 2014 we had our annual inspection, from which we had some violations. <br /> From the UST inspection report, we had a few Class II Violations. Violation 104 required us to <br /> upload Monitoring Site Plan, Certification of Financial Responsibility, UST Response plan, UST <br /> Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator. All of <br /> which were submitted into the California Environmental Reporting System on June 6th 2014, and <br /> I am sending a copy of the submittals that show they were submitted on the 6th of this month. <br /> Violation 302 and 304 were on the L4 annular sensor, which failed to activate an audible and <br /> visual alarm failed, but was fixed when the technician on site cleaned the sensor, which was then <br /> retested and passed. Violation 310 was when it was observed that there was one to three cups of <br /> liquid in the diesel STP sump. The technician on site removed the liquid from the diesel STP <br /> sump and therefore this violation was corrected. <br /> On the Hazardous Waste Program Inspection Report there were a couple class II <br /> violations. Violation 101, which states the location does not have a valid EPA ID. On the day of <br /> the Inspection we were given a California Hazardous Waste Permanent ID Number Application, <br /> which was filled out and faxed on May 91h 2014, I will have a copy of the application, and proof <br /> of reactivation as proof of the corrected violation. Violation 605, the 55-gallon metal container, <br /> at the time of the inspection the Hazardous Waste sticker had unreadable writing, which was <br /> later in the inspection fixed. To show proof I will send pictures of the tank. Due to some setbacks <br /> we have not been able to contact a licensed hazardous waste hauler to dispose of the waste, but <br /> as soon as possible I shall find one and send a copy of the manifest to the EHD. <br />
