(Cardno ATC), on your behalf for the above-referenced site. <br /> Cardno ATC proposes to destroy monitoring well MW-5 by pressure grouting as follows: <br /> ® Remove the well box/vault; <br /> • Excavate the subsurface soil approximately 24 inches in diameter around the well casing <br /> to 36 inches below ground surface(bgs); <br /> • Cut off and remove the well casing 30 inches bgs; <br /> • Utilize a pressure-grouting tool, affixed to the well casing, to insert neat cement grout <br /> slurry into the well; <br /> ® After grout extends to the top of the well casing, apply 25 pounds per-square-inch (psi) <br /> of air pressure to the well casing for approximately 5 minutes; and <br /> • Form a"mushroom cap" grout seal with at least 12 inches of neat cement grout poured 6 <br /> inches above and 6 inches below the top of the cutoff casing. <br /> Cardno ATC proposes to destroy MWA, MW-2, MW-3, MW-4, MW-6 and MW-7 by over-drilling <br /> as follows: <br /> Remove the well box/vault; <br /> Advance an auger that is slightly larger in diameter-than the original borehole into each <br /> well's entire length; <br /> Remove the well casing, sand pack, and other well components; and <br /> Backfill the resulting borehole with neat cement grout slurry mix using a tremie pipe to fill <br /> the borehole from the base to Surface grade. <br />
