Well Destruction Work Plan Pago 2 of 3 <br /> C&R International Sales Inc. November 24, 2014 <br /> 2403 Navy Drive <br /> Stockton, CA 95205 <br /> The EHD approves the well destruction methods proposed in the work plan with the following <br /> conditions: <br /> a Use only potable water to mix the cement grout; groundwater displaced from the wells or <br /> borings may not be used in grout or for any other purpose and must be properly <br /> containerized, labeled, stored and disposed; <br /> • Wells proposed to be over-drilled must be drilled out to their total depths; <br /> • Over-drilled wells' resulting boreholes shall be filled with neat cement, pumped from the <br /> bottom of the borehole up to 3 feet below surface grade (bsg) through a trernie pipe, <br /> extending to within 5 feet of the borehole's total depth; <br /> o For the well proposed to be pressure grouted, apply at least 25 psi pressure on the grout <br /> for 10 minutes; destroy the well by over-drilling if the well casing is unable to withstand <br /> this pressure for this length of time; <br /> o Following pressure grouting, enlarge the diameter of the top 3 feet of the borehole at <br /> least 1 foot larger than the auger originally utilized to install the well; cut off and remove <br /> the casing 6 inches above the bottom of the excavation before inserting the grout into <br /> the excavation for a thickness of 1 foot; <br /> o All Investigation Derived Waste(IDW), including all soil cuttings, groundwater, and water <br /> generated from well destruction activities, is required to be properly containerized, <br /> labeled, stored and disposed; <br /> • A well destruction report describing all well destruction field activities and including the <br /> signed manifests for the disposed IDW must be submitted to EHD. The EHD will issue a <br /> °No Further Action' closure letter once all wells have been properly destroyed, the <br /> signed waste manifests have been submitted, and all invoices have been paid; <br /> a Notify the EHD at least 2 weeks prior to implementing fieldwork so that an inspector can <br /> schedule to be present for the well destructions; and <br /> © The EHD requires that work may begin no sooner than 5 working days after the EHD <br /> permits have been issued. <br /> During the public participation period, which ended on 16 March 2014, EHD received no <br /> comments or concerns from interested individuals and agencies regarding the pending closure <br /> of this site. You may proceed with the well destructions if in compliance with the conditions <br /> noted above. <br /> To proceed with obtaining the destruction permit for these wells, the following must be <br /> submitted to the EFID to the attention of Gabriel Gonzalez: <br /> • El-ID Well and Goring Permit Application form; <br /> o Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); and <br /> • A fee of$1,300 (7 x $130 + 1 x $390). The permit fee for the destruction of each well is <br /> $130, along with a $390 Inspection Activity fee. <br />
