The disposal <br />receipt was incorrectly com eted with EPA ID# CAL000153470. CAL000153470 is assigned to the business at 600 <br />W. Lockeford St. Lodi, CA. II copies of each receipt shall contain all of the following information: <br />- The name, address, ide tification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative; <br />- The date of the shipme; <br />- The manifest number- <br />- The volume or quantity f each waste stream received, its California and RCRA waste codes, the waste stream <br />type listed in subdivision (c), nd its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/ A) identification number, if applicable; <br />- The name, address, an identification number of the authorized facility to which the hazardous waste will be <br />transported; <br />- The transporters name, address, and identification number; <br />- The driver's signature; <br />- A statement, signed by t e generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxici of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />Ensure that all future receipt for hazardous waste shipments using a consolidated manifesting procedure contain all <br />required information. <br />This is a minor violation. <br />117 <br />HSC 25404(e)(4) Failed to reort program data electronically. <br />A submission to the Californi Electronic Reporting System (CERS) for the hazardous waste program has not been <br />made. Beginning January 1, 013, all businesses are required to submit all new (or any changes to existing) <br />hazardous waste information online to the CERS at http-.//cers.calepa.ca.gov. Be sure to include your hazardous <br />waste activity in the Busines s Activities section in CERS in addition to any other relevant activities and required <br />fields. <br />This is a Class II violation. <br />601 <br />CCR 66262.34(d) Stored haz rdous waste onsite greater than 180 days. <br />The black 5 gallon container f hazardous waste metal fines has an accumulation start date of 8-9-11. This waste has <br />been on site longer than 180 ays. Facilities who generate less than 1000 kg of hazardous waste per month and do <br />not exceed 6000 kg of waste tored on site at anytime may store waste on site up to 180 days.
