LODI <br />May 05 2015 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />605 <br />CCR 66262.34(f) Failed to c mpletely label containers or tanks of hazardous waste. <br />The black 5 gallon container of hazardous waste metal fines has an accumulation start date of 8-9-11 and did not have <br />marked a hazardous wasteroperty. All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous aste" <br />- name and address of gE nerator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />During the inspection, the h ardous property of toxic was marked. Continue to ensure that all containers are marked <br />with all the required informat on. Since this container has been on site longer than 180 days, immediately contact a <br />licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the <br />EHD. <br />This is a repeat violation, Class II. <br />608 <br />CCR 66266.130 Failed to properly manage used oil and fuel filters. <br />Bills of lading for the recycling of used oil filters for 2012 were not found on site. A copy of each bill of lading must be <br />kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters for the last three <br />years, maintain them on site, and submit copies of 2012 to the EHD. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy ofthe <br />Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective act, <br />ns that have been or will be taken for each violation, and any supporting <br />paperwork, by 6-5-15. <br />Please be aware as of January <br />1, 2013, all businesses are required to submit all hazardous materials information <br />online to the California Environ <br />ental Reporting System (CERS) at http://cers.calepa.ca.gov. Be sure to include <br />your hazardous waste activity i <br />the Businesses Activities section in CERS in addition to any other relevant <br />activities and required fields. <br />Note: <br />The following issues were corre <br />ted during the inspection: <br />An emergency coordinator and <br />odified contingency plan information is lacking. There must be at least one <br />emergency coordinator on site <br />r on call to coordinate emergency response measures, and the following <br />information must be posted by <br />phone: the name and phone number of the emergency coordinator; location of fire <br />extinguishers, spill control equi <br />ment, and if present, fire alarm; and the phone number of the fire department, <br />unless t e facility has a direct all <br />rm.
