See attached "Continuation - Official Inspection Report' for violation(s) not listed above. <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br />Class II violations: (127, 605) <br />127. Copies of spent lead acid battery disposal records were not found on site. According to the facility <br />contact, the cores are taken at the same time that new batteries are purchased; the lead acid battery <br />receipts on site do not note cores taken. The generator shall retain on site for at least three years a <br />legible copy of each manifest or bill of lading which identifies spent lead acid batteries shipped to a <br />person who stores, uses, reuses, recycles, or reclaims the batteries or their components. Immediately <br />begin retaining a copy of disposal records for spent lead acid batteries and maintain them on site for <br />three years. <br />REPEAT VIOLATION <br />605. A 55 gallon black steel drum of oily debris was observed in the shop without a hazardous property or <br />accumulation start date. According to the facility contact, the drum was put in use for waste disposal <br />after Christmas 2011. All hazardous waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties (flammable, corrosive, reactive, toxic) <br />4. physical state (liquid, solid) <br />5. composition (contents) <br />6. accumulation start date <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE <br />
