(continued) <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all <br />the required information. <br />Notice to Comply - Minor violations: (116, 117) <br />116. Hazardous waste training is not being conducted annually. The hazard communication training <br />records on site are dated October 2010. The training program shall be completed by each employee <br />within six months of hire, prior to unsupervised work, and shall be reviewed annually. Immediately <br />conduct the required hazardous waste training and ensure that all employees receive this training at <br />least annually. Submit a copy of the training records to the EHD by April 1, 2012. <br />117. The training records on site were incomplete and included only a record that employees were <br />trained, with a basic overview of the training program. Training was not conducted by a person with <br />current hazardous waste management training. Facility personnel shall complete a training program that <br />teaches them to perform their duties in a way that ensures the facility's compliance with Title 22 <br />regulations. This program shall be directed by a person trained in hazardous waste management <br />procedures and shall include the following: <br />- hazardous waste management procedures relevant to the positions in which they are employed <br />- contingency plan implementation <br />- emergency procedures, equipment, and systems <br />- key parameters for automatic waste feed cut-off systems <br />- communications or alarm systems <br />- response to fire, explosion, or groundwater contamination incident <br />- shut down of operations <br />Training records shall be maintained on site until closure of the facility and shall include: <br />- name & job title of each employee <br />- written job description for each job title listed, including requisite skill, education, qualifications, and <br />duties <br />- description of training to be given to each position <br />- records that training was completed for each employee <br />Immediately prepare and implement a complete hazardous waste management training program for this <br />facility in accordance with Title 22 regulations and ensure that all employees complete this training. <br />Submit a copy of the updated training records to the EHD by April 1, 2012. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Rece By:, ., <br />Title: <br />Stacy Rivera (209) 468-3440 <br />EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
