An emergency coordinator and modified contingency plan information is lacking. <br />-- There must be at least one emergency coordinator on site or on call to coordinate emergency <br />response measures, and the following information must be posted by a phone: the name and phone <br />number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if <br />present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br />Immediately appoint an emergency coordinator and post the required information by a phone. <br />" A form is provided that can be used for this purpose. Submit proof of correction to the EHD by June <br />20, 2011 <br />60 & 63. Copies of hazardous waste disposal records were not found on site. <br />-- Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and <br />all receipts used in a consolidated manifesting procedure on site for three years and have them readily <br />available for review. <br />* Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, <br />and submit copies to the EHD by June 20, 2011. <br />NOTES <br />The facility generated less than 5 tons of hazardous waste in 2010 according to owner. <br />No Universal Waste was observed on site during this inspection, and owner stated he had not generated <br />any to date. Observed new universal fluorescent lamps stored out of box against the wall, and some <br />existing lamps still attached to fixtures that had been uninstalled and stored behind the paint booth. I <br />explain to the owner that if the lamps are broken he must manage the pieces as hazardous waste in <br />accordance, or make a hazardous waste determination and manage them accordingly. Also discussed <br />with owner management requirement of Universal Wastes including aerosol cans, lamps, alkaline <br />batteries, mercury containing switches, CRT, electronic devices, etc. in a closed container with a label <br />identifying the content and the start date of accumulation. Universal Wastes must be managed within 1 <br />year of accumulation, and the disposal records maintained on site for at least 3 years. <br />Submit the following to EHD by: June 20, 2011 <br />1) A completed copy of the (provided) Return to Compliance (please keep original for your records). <br />2) A written statement how each violation was corrected <br />3) Evidence (i.e. photos, disposal receipts, documents) in support of the corrections made. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THI FACILITY IS SUBJ CT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHDpecto <br />77L 11_�' <br />Received By: <br />� , ) C2 6111 a /� <br />Title_: <br />I <br />EHD f3 -02-00y fvfio ( / V CONTINUATION FORM <br />
