The following is an itemized list of hazardous waste violations that have not been <br />addressed for ACCURATE TRANSMISSIONS as of September 15, 2015. <br />Open violations from July 16, 2015 inspection <br />Violation #101 - Managed a hazardous waste without a valid EPA ID number. <br />This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or <br />offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate your <br />EPA ID number and submit evidence to the EHD. A form has been provided. <br />Violation #116 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on May 19, 2011 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by June 18, 201'. This <br />information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br />inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br />actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br />corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br />Violation #117 - Failed to report program data electronically. <br />A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br />made. Beginning January 1, 2013, all businesses are required to submit all new (or any changes to existing) <br />hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br />waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br />fields. <br />Violation #301 - Facility not maintained to minimize the release of a hazardous waste. <br />This is an outstanding violation from the inspection conducted on May 19, 2011 and the return to compliance has not <br />been submitted. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or releasE <br />of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately <br />clean the stain , and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting <br />documentation explaining how this waste was managed. <br />Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br />hazardous waste. <br />Observed 2-5 gallon buckets containing solvent nearby the part washer were found open. All hazardous waste <br />containers shall be closed at all times except when adding or removing waste. Immediately close these containers <br />and ensure all hazardous waste containers are closed when not adding or removing waste. <br />Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br />Observed the following: <br />1. 55 gallon white drum of used oil without the accumulation start date and the hazardous properties <br />2. 55 gallon black drum of used oil without the accumulation start date, physical state and hazardous properties <br />3. 2-5 gallon buckets containing solvent nearby the part washer without hazardous waste label <br />All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties (toxic, flammable, corrosive or reactive) <br />- physical state (liquid or solid) <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required information. SincE <br />it cannot be determined how long the 2-55 gallon drums (white and black) of used oil have been on site, immediately <br />contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifes <br />to the EHD. <br />Page 1 of 2 <br />
