Central Ave., Tracy <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br />"*Inspection conducted on November 18, 2010. Report completed on December 8, 2010. Checklist left <br />on site at time of inspection. <br />Notice to Comply - Minor violations: (8, 52, 60, 79, 83, 84, 85) <br />8. This facility does not have a valid EPA ID number to manage hazardous waste. A hazardous waste <br />generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br />an EPA ID number. Immediately obtain an EPA ID number to manage hazardous waste and submit <br />evidence to the EHD by January 8, 2011. A form has been provided. <br />52. An emergency coordinator and modified contingency plan information is lacking. At the time of <br />inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br />hazardous waste were properly trained. According to employees, no training has been conducted. <br />There must be at least one emergency coordinator on site or on call to coordinate emergency response <br />measures, and the following information must be posted by a phone: the name and phone number of the <br />emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; <br />and the phone number of the fire department, unless the facility has a direct alarm. The generator must <br />ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br />handling and emergency procedures. Immediately appoint an emergency coordinator and post the <br />required information by a phone. A form is provided that can be used for this purpose. Submit proof of <br />correction to the EHD by January 8, 2011. Immediately provide training to all employees who handle <br />hazardous waste and submit a copy of the training records to the EHD by January 8, 2011. <br />60. Copies of hazardous waste disposal records were not found on site. Hazardous waste generators <br />shall retain copies of all manifests signed off by the disposal facility on site for three years and have <br />them readily available for review. Immediately locate a copy of all manifests and receipts for the last <br />three years, maintain them on site, and submit copies to the EHD by January 8, 2011. <br />79. Waste fluorescent bulbs were observed stored open and uncontainerized. Universal waste lamps <br />shall be contained in a closed container or package that is structurally sound, adequate to prevent <br />breakage, and compatible with the contents of the lamp. Immediately containerize all universal waste <br />fluorescent lamps/bulbs in accordance with Title 22 regulations. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY- UBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />EHD I r j- <br />Received By: <br />Title: <br />_S <br />CONTINUATION FORM <br />
