Central Ave., Tracy <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />83. Waste fluorescent bulbs were observed unlabeled in the storage area. A generator shall label or <br />mark universal waste with the words 'Universal Waste' and the type of waste (alkaline batteries, mercury <br />containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans). Immediately label <br />these wastes and ensure that all universal wastes are labeled with the required information. <br />84. Facility was unable to demonstrate the amount of time used fluorescent bulbs were stored on site. A <br />universal waste generator shall accumulate universal waste for no longer than one year from the date <br />the waste was generated. Immediately dispose of this waste as universal waste and submit a copy of <br />the disposal record to the EHD by January 8, 2011. <br />85. Copies of universal waste disposal records were not found on site. A generator of universal waste <br />shall keep a record of each shipment of universal waste on site for at least three years. The record may <br />take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the <br />following: <br />- Name & address of destination facility <br />- Quantity & type of universal waste <br />- Date of departure/shipment <br />Begin this practice immediately. <br />Complete & submit a copy of the Return to Compliance Certification to the EHD by January 8, 2011 <br />along with any supporting paperwork and a statement documenting the corrective actions that have <br />been or will be taken for each violation. <br />"*Facility has been added as a hazardous waste generator under permit by the EHD. <br />Provided: <br />- Modified Contingency Plan template <br />- Schedule of FREE classes <br />- Return to Compliance Certification form <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY 1 UBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspe,jc1or <br />Received By: <br />Title: <br />EHD 23 -02 -OO R�bA 10/10 CONTINUATION FORM <br />
