SUMMARY OF VIOLATIONS (CLASS I. CLASS II. or MINOR -Notice to Comply) This is the final version of the Official Inspection Report This version supersedes the previous Checklist Only provided on 6/18/2014. This is a follow-up hazardous waste re -inspection. The following is a Class I violation: 204: 204 (Class 1). Observed a bottle of motor oil, aerosol can that's not empty, and a 5 -gallon bucket of flammable liquid disposal disposed to the trash collection area (See Photos #29, 30, 31, 32, 33), and leaking pink liquid which has been sampled and tested to be hazardous for acute toxicity in fish bioassay (see attached Sampling Photo and Sample Results). The disposal of any hazardous waste, or the causing thereof, is prohibited when the disposal is at a location which is not a permitted facility, or at any point which is not authorized. Immediately cease illegal disposal of hazardous waste to unauthorized point, contact a licensed hazardous waste hauler to dispose of the above-mentioned waste under manifest, and submit a copy of the manifest to the EHD by 9115/2014. The following are Class II violations: 101 108 116 122/125 128 301 304 404, 406, 407, 605/701 610/611 703 804/806/808/809. 812, and 9999. 101 (Repeat). This facility does not have a valid EPA ID number to manage hazardous waste at the time of the re -inspection. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately obtain an EPA ID number. Facility has since obtained an EPA ID on 7/7/2014; violation has been corrected. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
