Bills of lading for the recycling of used oil filters for the last three years were not found on site, and failed to return to compliance for the same violation cited in the 08/30/2013 Report. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the EHD by 9/15/2014. 301. Witnessed spillage of waste at "Gate T1" (Photos #5,7), "Building West" (Photo #15), outside "73" trailer (Photo #20), "Building North" (Photos #26), and "Building South" (Photos #37, 38, 39); also failed to return to compliance for the same violation cited in the 08/30/2013 Report.. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately clean up the spill and dispose of the wastes generated from the clean up in accordance to Title 22, and begin operating facility in a manner that will minimize the release of hazardous waste, and submit evidence of correction to the EHD by 9/15/2014. 304. Other than soda ash for acid spills, did not observe availability of spill control / decontamination equipment appropriate for the various hazardous wastes and materials stored at this facility; also failed to return to compliance for the same violation cited in the 08/30/2013 Report. Facility shall be equipped with portable fire extinguishers, fire control equipment, spill control equipment, and decontamination equipment for release of hazardous waste. Immediately provide spill control and decontamination equipment, and submit evidence of correction to EHD by 9/15/2014. 404. Witnessed multiple hazardous waste containers open (Photos #9,8,11,12, also Item #108 for any container determined to contain hazardous waste) during this inspection, and failed to return to compliance for the same violation cited in the 08/30/2013 Report. Hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. Submit evidence of correction to EHD by 9/15/2014. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
