Report completed on January 20, 2011. Checklist left on <br />site at time of inspection." <br />Class I I violations: (18, 20, 21, 23, 27, 31, 45, 49, 60, 61, 62, 63, 83, 84, 85) <br />18 & 20. A 55 gallon drum of used absorbent was observed in the outside storage area with a faded <br />label on which only the words 'hazardous waste' and the contents were legible. Two 20 gallon drums of <br />used antifreeze (according to Mr. Shannon) were observed in the shop without labels. All hazardous <br />waste containers shall be marked with the following information: <br />1. the words "Hazardous Waste" <br />2. name and address of generator <br />3. hazardous properties <br />4. physical state <br />5. composition (contents) <br />6. accumulation start date <br />Immediately label these containers and ensure that all containers are marked with all the required <br />information. Facilities who generate 1000 kg or more of hazardous waste per month or exceed 6000 kg <br />of waste stored on site at any time may only store waste on site up to 90 days. Since it cannot be <br />determined how long the 55 gallon drum of used absorbent and the two 20 gallon drums of used <br />antifreeze have been on site, immediately contact a licensed hazardous waste hauler to dispose of this <br />waste under manifest and submit a copy of the manifest to the EHD by February 20, 2011. <br />REPEAT VIOLATION <br />21 & 23. An uncovered 55 gallon drum and a dumpster of used oil and fuel filters were observed without <br />labeling. Used oil filters must be stored in a closed, rainproof container, labeled as "drained used oil <br />filters," and marked with an accumulation start date. Containers of of oil filters commingled with gas/ <br />diesel filters shall be stored in containers designed to prevent ignition of the fuels and labeled "used oil <br />and gasoline filters." Immediately label and close these containers and ensure that all used oil filters are <br />managed in this way. <br />REPEAT VIOLATION <br />ALL EMD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Received y: <br />Titl <br />I <br />EHD 23-09-003 ev 08/10/10 CONTINUATION FORM <br />
