Two 20 gallon drums of used antifreeze (according to Mr. Shannon) were observed without lids in <br />the shop. All hazardous waste containers shall be closed at all times except when adding or removing <br />waste. Immediately close these containers and ensure all hazardous waste containers are closed when <br />not adding or removing waste. <br />REPEAT VIOLATION <br />31. A tank assessment was not available for the used oil tank. A hazardous waste generator shall <br />obtain and keep on site a written assessment for each hazardous waste tank at the facility. This <br />assessment shall be reviewed and certified by an independent, qualified, professional engineer, <br />registered in California, and shall attest to the tank system's integrity. The assessment shall determine <br />that the tank system is adequately designed and has sufficient structural strength and compatibility with <br />the waste stored, and shall be written in accordance with Title 22 regulations. Immediately obtain a <br />written tank assessment for all hazardous waste tanks at this facility in accordance with Title 22 <br />regulations, and submit a copy of the certified assessments to the EHD. <br />45. The contingency plan for this facility did not include descriptions of emergency response to fire or <br />explosion, a list of emergency equipment at the facility, the phone number for Cal EMA, or the name, <br />address, and phone number of the facility Emergency Coordinator. A contingency plan shall include all <br />of the following: <br />-- actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br />or surface water <br />-- arrangements agreed to by local police departments, fire departments, hospitals, contractors, and <br />state & local emergency response teams <br />-- names, addresses & phone numbers (office & home) of the Emergency Coordinator <br />-- list of all emergency equipment at the facility (fire, spill, communications/alarms, decon), including <br />location, physical description, and a brief outline of its capabilities <br />-- evacuation plan, including routes and signals to be used <br />-- telephone number of Cal EMA <br />This information must be kept current at all times.
