Lowell <br />Tracy, CA 95356 <br />Pursuant to the San Joaquin Local Health District fee schedule Resolution No. 85- <br />58, a $35.00 consultation fee shall be submitted along with the completed general <br />Environmental Health Application Form for review of the above referenced <br />environmental assessment/remediation activity. <br />6 <br />Each additional hour over the initial one hour of time spent in review activity <br />will be billed at the rate of $35.00 per hour. The additional amount due will be <br />billed to the applicant identified on the general Environmental Health Application <br />Form at the completion of requested activity, or monthly, if longer than 30 days is <br />required for completion. <br />Should you have questions concerning this matter, contact Diane M.
