Section 66 265.54, Division 4.5, Title 22, California Code of Regulations: At the <br />time of inspection, the contingency plan did not reflect a personnel change for the <br />emergency coordinator. <br />8. Section 66268.7 (a)(6), Division 4.5, Title 22, California Code of Regulations- 21 <br />land disposal restriction notification form could not be found for manifest <br />#88456353. 1 <br />9. Section 66265.16, Division 4.5, Title 22, California Code of Regulations: <br />Employee training in hazardous waste management procedures was inadequate. <br />H. SCHEDULE OF CORRECTIONS: <br />Violations #1 Effective immediately, all hazardous waste containers shall be <br />#2, #3, and #4: labeled with the following information: <br />a) The start date of accumulation. <br />c) Composition and physical state of the water. <br />d) Statement(s) which will call attention to the particular <br />hazardous properties of the waste. <br />e) Name and address of person producing the waste. <br />Violations Effective immediately, all hazardous waste containers shall be closed <br />#5 and #6: except when adding or emptying waste material. <br />Violation #7: Effective immediately, the contingency plan shall be completed <br />according to referenced code section. <br />Violation #8: Effective immediately, attach all Land Ban Certificates to applicable <br />hazardous waste manifests. <br />Violation #9: Within 30 days of receipt of this report, complete and maintain <br />employee training records. <br />2 <br />
