California 95205-6232 <br /> Telephone: (209)468-3420 Fax: (209)468-3433 Web:www.sigov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> S B GAS & MARKET 515 W ELEVENTH ST, TRACY July 15, 2015 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The 001 stand alone sensor in the UDC 7/8 sump failed to stop the flow of product at the dispenser when tested. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. The technician retested after cleaning the sensor and it still failed to stop the flow of <br /> product at the dispenser. Correct immediately by having a properly licensed, trained, and certified contractor replace <br /> the failed component with a functional component(LG 113-x listed, if applicable) and obtain a permit within one <br /> business day from the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by August 14, 2015. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance, The UST Owner/Operator: Written <br /> Agreement, if applicable, can be stored at the facility or uploaded into CERS. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS F CILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Received By Received By(printed name and title): Date: <br /> (signature): Ken Cook, Assistant Manager "5 <br /> Inspector Inspector(printed name and title): In pector Phone: <br /> (signature): CINDY VO, EH Specialist (209)468-8257 <br /> Page 6 of 6 <br /> I <br />
