Aek low <br /> San Joaquin County <br /> Environmental Health Department <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone: (209)468-3420 Fax:(209)468-3433 Web:www.sjgov.org/ehd <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> S B GAS & MARKET 515 W ELEVENTH ST, TRACY July 15, 2015 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 126 HSC 25404(e)(4) Failed to report program data electronically. <br /> The UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST <br /> Owner/Operator Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of <br /> Designated UST Operator Compliance were not submitted to the the California Electronic Reporting System (CERS). <br /> Beginning January 1, 2014, all businesses are required to submit all new (or any changes to existing) UST <br /> information, including: UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, <br /> UST Owner/Operator Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement <br /> of Designated UST Operator Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure to include any <br /> other relevant activities and required fields. <br /> This is a repeat violation, Class II. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> The monitoring system certification for 2014 was not found on site at the time of inspection. These records shall be <br /> maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; <br /> (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of <br /> sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, <br /> ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Ken Cook, assistant <br /> manager, called the service company and obtained a copy of the monitoring system certification test results before <br /> the end of the inspection. This was corrected on site. <br /> This is a minor violation. <br /> 209 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Nasratullah Amari was not performed and Ken Cook was not current. <br /> Ken was last trained on February 24, 2014. The designated operator shall train facility employees for which he or she <br /> is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall <br /> include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
