HIGHWAY 99, STOCKTON, SAN JOAQUIN COUNTY <br /> On 29 September, 28 October and 3 November 2003, Ground Zero Analysis, Inc. (GZA) submitted a <br /> Report of Waste Discharge (RWD), associated information, and applied for coverage under Waste <br /> Discharge Requirements General Order No. R5-2003-0044 (General Order) on your behalf for your site at <br /> 8200 N. Highway 99, Stockton. The RWD and supporting technical information submitted for an on-site <br /> discharge of treated groundwater from your facility were determined to be complete on 29 October 2003. <br /> The RWD is in support of a discharge of treated groundwater from a groundwater extraction and <br /> treatment system located at your site. The treatment system has been designed to remove petroleum <br /> hydrocarbon contaminants, including methyl tertiary butyl ether(MTBE), from the extracted <br /> groundwater. This treated groundwater will then be discharged to land downgradient of the existing <br /> contaminant plume on the site. <br /> The General Order includes a Monitoring and Reporting Program (MRP) that prescribes minimum <br /> wastewater monitoring requirements. The MRP requires sampling and analytical testing for specific <br /> petroleum hydrocarbon constituents in influent, effluent and monitoring wells. Additional and/or <br /> modifications to the MRP are included in the Additional Requirement section of this Notice of <br /> Applicability(NOA). <br /> Based on our review of the information provided, we have determined this project meets the required <br /> conditions to be approved under the General Order. All requirements contained in the General Order <br /> and/or modified by this NOA are applicable to your project. You are hereby assigned General Order <br /> No. R5-2003-004412 to regulate the discharge of treated groundwater at the Speedy Foods site. <br /> PROJECT LOCATION <br /> The Speedy Foods site is at 8200 N. Highway 99, Stockton, California.
