Hwy 99 <br /> Acampo, CA <br /> On September 10 , 1993 , Park Environmental Corporation, started <br /> over-excavating the former underground storage tank pit at the <br /> above referenced site according to the proposed work plan dated <br /> August 8 , 1993 . Within that work plan proposal, the scope of work <br /> included documenting findings . On October 21, 1993 , Park <br /> Environmental conducted additional excavation of the petroleum <br /> contaminated soil . <br /> On November 15, 1995, Park Environmental sent a facsimile of the <br /> results of the soil samples collected during the two phases of <br /> excavation. After reviewing the areas excavated and the areas of <br /> known contamination evidenced by the soil borings drilled in July <br /> 1992 , PHS/EHD staff verbally informed you and Park Environmental <br /> that further excavation and soil sampling was required. <br /> On November 23 , 1993 , a soil sample was collected from the bottom <br /> of the excavation at about 16 . 5 feet below grade and from the east <br /> sidewall of the excavation at about 11 feet below grade by Park <br /> Environmental . There was odor to both of these soil samples . The <br /> results of the analysis of those two soil samples have never been <br /> submitted to PHS/EHD. <br /> On February 11, 1994, further excavation to a depth of 24 feet <br /> below grade was conducted and Park Environmental collected 3 soil <br /> samples during that phase of work. The results of the analysis of <br /> those three samples have never been submitted to PHS/EHD either. <br /> Although, PHS/EHD staff was verbally informed by Park Environmental <br /> that the results were higher than previous soil sample results . <br /> On March 8 , 1994 , three more soil samples were collected by Park <br /> Environmental from the excavation. The bottom sample was mixed in <br /> a bucket and then split into two samples that were to be sent to <br /> two different laboratories . The results of the analysis of these <br /> samples were never submitted to PHS/EHD, but PHS/EHD was informed <br /> verbally that contamination was found. <br /> A Division or San Joaquin County Health Care Services <br />
