I Les Calkins Page 2 On November 3 , 1994, Geological Audit Services, Inc . collected three soil samples from the bottom of the excavation at depths of 25 . 5 to 27 . 5 feet below grade. :The results of those soil samples were submitted in a proposal to excavate additional soil, since one of the samples evidenced 4800 parts per million of total petroleum hydrocarbons as diesel . On November 29, 1994, excavation activities continued to a depth of 36 . 5 feet below grade . Geological Audit Services, Inc . collected E 5 soil samples after this phase of excavation (one bottom sample and 4 sidewall samples) . The results of the. analysis of these samples have never been submitted to PHS/EHD. On April 14 , 1995, Advanced GeoEnvironmental, Inc . collected soil Sample results were samples from the soil stockpiled at the site. p to be used to propose remedial alternatives for the soil . PHS/EHD received a facsimile of those results on April 20, 1995 and total IE petroleum hydrocarbons as diesel was present . in all samples collected. On April 25, 1995 , the final excavation of contaminated soil was conducted and samples were collected from this area of excavation on April 26 , 1995 . Four soil samples were collected at depths of 24 to 28 feet below grade . These sample results have never been submitted to PHS/EHD. It was discussed at this time with Advanced GeoEnvironmental and your representative, Mark Rosers, that a complete report, signed by a properly registered or certified professional, of all the excavation activities and soil sample results since September 10, 1993 , was to be submitted to PHS/EHD. It was also discussed at that time:, that the clean soil generated from ,the installation of a storm pond at the site could be used to fill the excavation. On May 5, 1995, PHS/EHD staff conducted a site visit ., There was still 2 to 4 feet of fill required to bring the area to ,grade . PHS/EHD staff approved of lining the area with plastic and., placing contaminated soil on top of the plastic to bring the area to grade . This was with the understanding that this 2 to 4 feetlof soil would be remediated and confirmation sampling conducted, at the same time that the other contaminated stockpiled soil was remediated and sampled. On November 10, 1995, PHS/EHD 'staff- conducted a site visit and discussed with you that a report of all the activities had not been submitted to this agency. You stated you would discuss this with Mark .Rosers . Please be advised that Mark Rosers does not have the registration or certification required in order to take responsibility for the technical information required in the report of the work that was conducted at your site . In addition, you may not be reimbursed for any costs for his services by the State Water Resourses Control Board Cleanup Fund Program (Cleanup Fund) .
