I <br /> Les Calkins <br /> Page 2 <br /> On November 3 , 1994, Geological Audit Services, Inc . collected <br /> three soil samples from the bottom of the excavation at depths of <br /> 25 . 5 to 27 . 5 feet below grade. :The results of those soil samples <br /> were submitted in a proposal to excavate additional soil, since one <br /> of the samples evidenced 4800 parts per million of total petroleum <br /> hydrocarbons as diesel . <br /> On November 29, 1994, excavation activities continued to a depth of <br /> 36 . 5 feet below grade . Geological Audit Services, Inc . collected <br /> E 5 soil samples after this phase of excavation (one bottom sample <br /> and 4 sidewall samples) . The results of the. analysis of these <br /> samples have never been submitted to PHS/EHD. <br /> On April 14 , 1995, Advanced GeoEnvironmental, Inc . collected soil Sample results were <br /> samples from the soil stockpiled at the site. p <br /> to be used to propose remedial alternatives for the soil . PHS/EHD <br /> received a facsimile of those results on April 20, 1995 and total <br /> IE petroleum hydrocarbons as diesel was present . in all samples <br /> collected. <br /> On April 25, 1995 , the final excavation of contaminated soil was <br /> conducted and samples were collected from this area of excavation <br /> on April 26 , 1995 . Four soil samples were collected at depths of <br /> 24 to 28 feet below grade . These sample results have never been <br /> submitted to PHS/EHD. It was discussed at this time with Advanced <br /> GeoEnvironmental and your representative, Mark Rosers, that a <br /> complete report, signed by a properly registered or certified <br /> professional, of all the excavation activities and soil sample <br /> results since September 10, 1993 , was to be submitted to PHS/EHD. <br /> It was also discussed at that time:, that the clean soil generated <br /> from ,the installation of a storm pond at the site could be used to <br /> fill the excavation. On May 5, 1995, PHS/EHD staff conducted a <br /> site visit ., There was still 2 to 4 feet of fill required to bring <br /> the area to ,grade . PHS/EHD staff approved of lining the area with <br /> plastic and., placing contaminated soil on top of the plastic to <br /> bring the area to grade . This was with the understanding that this <br /> 2 to 4 feetlof soil would be remediated and confirmation sampling <br /> conducted, at the same time that the other contaminated stockpiled <br /> soil was remediated and sampled. <br /> On November 10, 1995, PHS/EHD 'staff- conducted a site visit and <br /> discussed with you that a report of all the activities had not been <br /> submitted to this agency. You stated you would discuss this with <br /> Mark .Rosers . Please be advised that Mark Rosers does not have the <br /> registration or certification required in order to take <br /> responsibility for the technical information required in the report <br /> of the work that was conducted at your site . In addition, you may <br /> not be reimbursed for any costs for his services by the State Water <br /> Resourses Control Board Cleanup Fund Program (Cleanup Fund) . <br />
