II. SCHEDULE OF CORRECTIONS: <br /> Violation 91 Effectively immediately, all hazardous waste <br /> collected at points of generation exceeding the <br /> quantity limit shall be relabeled with a new <br /> accumulation start date and moved to the hazardous <br /> waste storage area. <br /> Violation #2 : Within 10 days of receipt of this report, all <br /> hazardous waste containers used at the point of <br /> waste generation shall be properly labeled with the <br /> following information: <br /> a) The accumulation start date. <br /> b) The words "Hazardous Waste" . <br /> c) Composition and physical state of the waste. <br /> d) Statement(s) which will call attention to the <br /> particular hazardous properties of the waste. <br /> e) Your name and address. <br /> Violation #3 : Effective immediately, all containers for hazardous <br /> waste shall be closed except when adding or <br /> emptying waste. <br /> Violation #4 : Within 10 days of receipt of this report, employee <br /> training records shall be updated to include the <br /> job title of each position at the facility related <br /> to hazardous waste management, and the name of the <br /> employee filling the position. <br /> Violation #5 : Within 10 days of receipt of this report, employee <br /> training records shall be updated to include a <br /> written job description for each position listed <br /> under section 66265 . 16 (d) (1) . <br /> When all viol tions have been corrected, please send written <br /> certificationo this office at the letterhead address. This <br /> certificationhall contain the sample certification statement <br /> indicated on At achment I. <br /> PHS-EHD may cojDduct a follow-up inspection at the facility to <br /> verify compliance. <br /> The issuance of this report of violation and schedule of correction <br /> does not preclude PHS-EHD from taking any administrative, civil, or <br /> criminal actionasa result of the violations noted herein. <br /> 2 <br />
