SCHEDULE OF CORRECTIONS <br /> Violation #1: Effective immediately, employee training records shall be in compliance <br /> with referenced code section. <br /> Violation #2: Effective immediately, all Land Disposal Restriction (LDR) paperwork shall <br /> be kept with accompanying manifest. <br /> Violation #3: Effective immediately, all drums containing hazardous waste shall be <br /> labeled with the following information: <br /> (a) The accumulation start date. <br /> (b) The words "Hazardous Waste". <br /> (c) Composition and physical state of the waste. <br /> (d) Statement or statements which will call attention to the particular <br /> hazardous properties of the waste. <br /> Violation #4: Effective immediately, all used oil filters shall be handled in accordance <br /> with referenced code section. <br /> Violation #5: Effective immediately, all hazardous waste manifests for the past three <br /> years shall be available for PHS-EHD review. <br /> When all violations have been corrected, please send written certification to this office at the letterhead <br /> address.
